Bring the right questions to your doctor’s appointment
Remembering what you wanted to ask after the appointment? Prepare a clear conversation sheet with your observations and most important questions.

A prompt to try
Help me only to prepare for a doctor’s appointment that is already scheduled. Do not provide a diagnosis, urgency assessment or treatment recommendation. Reason for the appointment in my own words: [REASON] What I have observed and since when: [OBSERVATIONS] Questions I have already noted: [QUESTIONS] What matters most to me in the conversation: [GOAL] Existing documents I want to bring: [LIST WITHOUT PERSONAL DATA] Ask up to three organizational questions if needed. Put my observations into a short timeline without adding causes. Separate observations from my guesses. Suggest no more than five questions, starting with my main concern. Questions about proposed measures may cover benefits, possible downsides, alternatives and next steps for the treating clinician to answer; do not answer these medical questions yourself. Ask me to choose the three most important questions. Then create a compact conversation sheet: my concern in two sentences, observations, the three selected questions, documents to bring and blank spaces for answers and next steps. Do not invent findings, change medication or infer reassurance from this sheet.
